I'm not seeing anyway to add new categories beyond the existing "general" category that exists by default in my UNA.io installation. The control panel section under discussions for categories is where I would expect to see a button to add, but none is there. See attached screenshot.
You have to create them yourself.
Studio -> Forms -> Data Items -> Select Module -> Discussions Categories -> Add New Item
Then you can order them accordingly by clicking the 3 bars to the left and dragging them up or down.

Going there, I see "Discussions" but not "Discussion Categories" and the "Add New Item" for "Discussions" is greyed out. See attached image.
Yes, first you select the module, and then the list. Try to select discussions, and then Categories in the second drop down. This tutorial should help - How to change categories in content apps?

That worked a treat - but I don't see the categories when logged in as a user. Do I need to enable it somewhere?

If you mean Categories field then this visibility can be corrected via Studio->Forms->[necessary module]->[Necessary form] area. In what page you don't see it?
Sorted, turned I needed to creat a forum psoting for the catgeories to appear for users.
