Error with Videos

Not sure exactly what happened but here's the error...

Database error in We Build Together

Query:
INSERT INTO `sys_transcoder_queue` SET `transcoder_object` = ?, `profile_id` = ?, `file_url_source` = ?, `file_id_source` = ?, `status` = ?, `added` = ?, changed = ?

Mysql error:
Duplicate entry 'bx_timeline_videos_webm-20' for key 'transcoder_object'

Location:
The error was found in query function in the file /var/www/vhosts/fiveforchange.org/httpdocs/inc/classes/BxDolTranscoderQuery.php at line 193.

I have no idea as I think it was one of the users that created it. Not sure but it might be due to vertical videos being uploaded.

I've tried to reproduce it using different ways and this problem isn't reproducible on my end. Please let me know if you figure out how to reproduce it ?
