Updating My Cover Image Here Resulted in Empty Black Box?

I tried to update my cover image and now it is only a black box? The image desired is shown below, so I know UNA can display it.

I am seeing the same issue on my site. Either black completely, or non centered pics. Not sure what it is. I have also tried multiple images on my temp profiles. It took 4 tries to finally get one of them to take. then the other profile took it just fine. Weird issue.
