Creating and integrating a MAILTO form

I am looking at making a mailto form to get feedback from my members and wanted to know A. Can this be done in UNA and B. What are the steps to put this together. What Im looking to do is build a form that would have possibly some drop down options to make choices and a body for them to type feedback in and then have a send button to mail the form to me at a certain address. Then I would want to present the form to my members either via a separate page or link embed it in a post. If anyone could give some guidance on doing this in UNA I would greatly appreciate it!
